When patients come from areas that necessitate long-distance travel, the ONN partners with the patient, primary oncologist, and community resources for necessary supportive care infusion services, thereby improving treatment compliance, safety, and the patient’s quality of life. The ONN will be the bridge to outpatient by ensuring excellent communication to the ambulatory providers/staff and affording the patient a known and trusted contact after hospital discharge. 

By providing the patient with a well-coordinated transition to vital services, the ONN can decrease 7-day hospital readmissions rates and positively impact treatment compliance and thereby clinical outcomes. At University of California, Davis Medical Center a downtrend in length of stay and mortality have been noted since ONN program began.
